The Aboriginal Patient Navigator assists and supports patients/families/community members both in the hospital and community in accessing culturally safe, culturally appropriate and culturally sensitive health care services and supports. The Aboriginal Patient Navigator works with all health care teams (hospital/community – on and off-reserve) which includes our patients/families/community members to ensure culturally safe, culturally appropriate and culturally sensitive health care services and supports are provided and received. Referrals to the Aboriginal Patient Navigator can be made by the patient, family, community members, nurse, doctor or any of your Health Care Team involved in your Circle of Care.
