Medical Patient Transportation Program

Important Notice

Sorry for any inconvenience, Splatsin Health Services will NOT have a Medical Transportation driver available until further notice.

In the meantime please follow regular Purchase Order process for any medical appointments:

• Contact Medical Transportation Coordinator with any medical appointments
• Pick up confirmation of attendance form from Health Centre
• Bring confirmation back to the Health Centre for reimbursement (Purchase Order) of fuel after you attend your medical appointment
