PURPOSE AND KEY FOCUS

We are looking to fill this important role within our Community Health Division. The Home Care Aide is part of a multi-disciplinary team delivering health care support to the Splatsin community. remain safely in their own homes.  The Home Care Aide is responsible for providing quality home care services to clients in their homes, advocate for clients who need supports in accessing additional community resources.

JOB SUMMARY

The Home Care Aide provides individualized care and coordinates resources to clients who, persons with disabilities and others so that they may continue to live in their homes. Home care may include transportation, house cleaning, personal hygiene, providing meals, and other health and wellness related activities.

The Home Care Aide is also responsible for delivering the Home Care Program. They will maintain confidential client files and will prepare and present reports and information as required. The Home Care Aide will ensure that all home care is provided in a caring and respectful way in keeping with all relevant policies, procedures and Splatsin home care standards.

The Home Care Aide will liaise with other community services and resources to ensure that additional resources are involved, if required. They will also provide information and education to individuals and groups in the community as required and requested.

Reports to the Home Community Care Nurse

JOB DUTIES AND RESPONSIBILITIES

  • Provides individual home care to elders, persons with disabilities and others in the community with identified needs
  • Provides home care including general housekeeping, personal hygiene and simple meals or snacks
  • Transports home care clients as coordinated by the HCCN when family is unavailable
  • Assists with exercises, physiotherapy and other medical tasks assigned by the Home Care Nurse or under the direction of medical service providers
  • Monitors blood pressure and other physical conditions as required under supervision
  • Follows medical instructions carefully and consistently
  • Organizes time and resources based on the individual needs of home care client
  • Manages any unanticipated events as directed by the supervisor and within scope of practice
  • Delivers the Home Care Program in an appropriate, caring, and respectful manner
  • Identifies persons requiring home care and directs individuals and their families to appropriate programs
  • Coordinates appropriate care and equipment including other community resources as required
  • Consults with family members and other supports to ensure that care is on-going and that all client needs are identified and met
  • Assesses fall and frailty risk
  • Encourages clients and families to be involved in care andtake responsibility for care
  • Liaises with all family, medical and other resources
  • Advocates on behalf of clients for additional services and resources
  • Participates in community events as requested

ADMINISTRATIVE DUTIES

  • Provides information to other health care professionals, as required
  • Records home care services in confidential client file after each visit
  • Educates clients, families, and support systems about the Home Care Program – what can be provided and when to access other resources
  • Reports any concerns to the supervisor
  • Monitors supplies and resources
  • Attends meetings
  • Ensures adherence to Home and Community Care Policy Manual, the Contribution Agreement from Health Canada, Indigenous Services Canada policies and procedures
  • Performs all duties and responsibilities in accordance with the Splatsin policies and procedures
  • Performs other duties as required

QUALIFICATIONS

Education

  • High School Diploma/GED
  • Certified Care Aide Certificate
  • Current First Aid/CPR certificate with defibrillator or working toward completion
  • WHMIS and Foodsafe certificates or working toward completion
  • Valid BC Driver’s license, minimum Class 5 and reliable vehicle
  • Clean Driver’s Abstract

Experience

  • Experience in health-related field
  • 2 years of client support experience, ideally in a community-based health services environment
  • Experience working with First Nations communities

Commensurate equivalent to experience
